5. Emergency Medical Authorization

In a serious or uncertain medical situation, SCCFO may call 911 and contact the adults listed on this form. SCCFO does not provide medical services.

I authorize SCCF and SCCFO to give relevant information to emergency responders and licensed health care professionals, request emergency evaluation and ambulance transport when appropriate, and permit licensed emergency personnel and health care professionals to provide reasonably necessary care if I cannot be reached promptly.

I understand that the Participant or family remains responsible for medical and transportation expenses except to the extent an insurer pays them or SCCF is legally responsible.

6. Media Permission

Photos and recordings help SCCF and SCCFO celebrate our musicians and share the program with families and the community. We hope you will grant permission, but your choice does not affect membership.

I give permission for SCCF and SCCFO to photograph or record the Participant during program activities and use the Participant’s image, voice, and performance in websites, social media, newsletters, printed materials, and other nonprofit promotional or educational materials.

I do not give permission. SCCF and SCCFO will make reasonable efforts not to feature the Participant in identifiable publicity photographs or recordings.

A media choice may be changed for future use by emailing events.sccf@gmail.com. SCCF and SCCFO cannot control recordings made by audience members, host organizations, or other third parties at public events.
